The Compass Care program consists of an interdisciplinary team of providers that help to coordinate, support, and assist individuals who have been diagnosed with a Substance Use Disorder and have Medicaid.
The Compass Care service is customized for each individual and may consist of a combination of services including medical, mental health, substance use disorder treatment, peer recovery, and community resource services to provide person-centered care. Each individual enrolled will have a treatment team to help deliver services.

The individual needs and goals of each participant will help determine who serves as a member of the treatment team. The team may include a combination of:
The assigned lead will work with the client to identify social determinants of health as well as behavioral and physical needs used to identify goals for treatment. All care coordination is designed to be delivered in a flexible manner best suited to the individual's preferences and to support goals that have been identified in the care plan.
Services within the Compass Care program work to educate and engage individuals in making decisions that promote a healthy lifestyle. This may include providing patient-centered training, encouraging routine preventative care, and using evidence-based practices to help clients manage their care.
Goals and treatment needs will be reviewed by the team monthly. Services for each individual participant will vary based upon the needs and goals identified, and clients preference. Whether an individual benefits from a monthly review or multiple service provisions, GLRC remains committed to empowering each individual's recovery journey through this coordination of care!
